Advocate Health Care and LifeBridge Health Improve Patient Access by Scaling HealthLoop Enterprise Wide


Advocate Health Care, the largest health system in Illinois, and LifeBridge Health, one of the largest providers of health-related services in Maryland, recently expanded their scope of services with HealthLoop, the leading patient engagement software solution.

HealthLoop's solution enables care teams to engage all patients before and after admission through automated daily check-ins. By sending the right information at the right time, HealthLoop identifies those patients that need help in real-time, allowing care teams to proactively intervene before costs and complications escalate. Advocate Health Care initially deployed HealthLoop in early 2016 across ten orthopedic and spine practices in their network. This past December they chose to scale HealthLoop throughout all surgical episodes and an initial roll-out in support of their care management programs for chronically ill patients. Advocate completed an internal assessment of the outcomes and identified a statistically significant impact on complications, re-admissions, and total episode cost reduction while also receiving positive feedback from participating physicians and staff.

LifeBridge Health partnered with HealthLoop in July 2015 in a development agreement focusing on pre-admission and post-discharge support for orthopedic patients at the Rubin Institute for Advanced Orthopedics at Sinai Hospital. As a result, the practice saw a 50 percent reduction in inbound calls related to care, a reduction in readmissions and complications, an increase in CG-CAHPS, and an overall 93 percent of patients using HealthLoop are "Extremely Likely to Recommend" their provider. Based on their outcomes, LifeBridge Health chose to expand HealthLoop enterprise-wide in December.

In addition to bringing on several new customers, HealthLoop has recently released its latest data which indicates continued increases in patient satisfaction, steady improvements in care management performance benchmarks, and the reduction of readmissions and complications. As of January 2018, HealthLoop continues to show that 92 percent of patients using their solution are "Extremely Likely to Recommend" their provider to a friend or colleague. Providers utilizing HealthLoop have witnessed a 54 percent drop in complications, a 45 percent reduction in readmissions, and an average costs savings of $656 per episode. Moreover, HealthLoop customers report that 74 percent of their patients complete patient reported outcome measures (PROMs) over 12 months, enabling them to capture additional bonus payments and earn incentives under Merit-Based Incentive Payment Systems (MIPS) mandated by CMS.
