APIs push care plans to point of care HealthE Connect uses APIs to push the auto-generated care plans to the EHR at the point of care to keep the provider apprised of the healthy activities and quality measure screenings completed by the patient at the next face-to-face visit. Episode of Care Management Pre-op/post-op care plans and education give patients and their family members tangible resources to go home after an inpatient encounter.
