Mobile Care Chicago Wins Premier Cares Award and $100,000 for Community Initiative


Mobile Care Chicago, a not-for-profit organization providing medical care to low-income children via mobile clinics, with a focus on Chicago's far South side, has received the 25th annual Monroe E. Trout Premier Cares Award.

The award, which includes a $100,000 cash prize, celebrates Mobile Care Chicago's Roseland Community Initiative, a program that provides education and medical care to low-income children diagnosed with the chronic disease asthma. The Initiative used asthma educators stationed within a local hospital emergency department, as well as local schools, to educate families on children's respiratory issues and how to best manage them. Mobile Care Chicago's initiative also manages an Asthma Van that delivers ongoing, comprehensive medical care to effectively manage asthma and severe allergies to patients directly in their communities. As a result of these combined efforts, Mobile Care Chicago has successfully reduced pediatric asthma-related emergency department utilization at Roseland Community Hospital by more than 80 percent.

"Unmanaged chronic conditions make up the bulk of soaring healthcare costs," says Matt Siemer, executive director of Mobile Care Chicago. "But translating that acknowledgement into action is an incredible challenge because the barriers low-income families grapple with are things like transportation, erratic work schedules and lack of resources. Our hypothesis was that if we provided a convenient mobile specialty practice to families through local schools, and especially ocused on reaching kids who weren't able to access preventative care elsewhere, we could provide a better quality alternative and lower the number of people dependent on their local hospital for emergency care. What we found was that for virtually all our patients, the Asthma Van worked miracles. We're eager to share our findings and do our part to make sure all kids with asthma can benefit from programs like this one."



Mobile Care Chicago's Asthma Van is a specialty mobile clinic equipped with diagnostic equipment and medications designed to treat children with asthma and severe allergies in their own neighborhoods. Asthma educators offer home environment assessments to families and provide case management as needed. The Asthma Van began rotating through nine schools in the Roseland neighborhood for the Initiative, screening more than 3,500 children for respiratory disease symptoms. All services and medications are offered at no cost to families.

A panel of national healthcare leaders selects the Premier Cares Award winner and two finalists, all of which receive cash awards for use in further improving their programs. The Cares Award program spotlights these community-based healthcare initiatives and helps other organizations learn to replicate the unique programs.


Siemer, along with Dr. Philip Sheridan, founder and Board Member of Mobile Care, were recognized on June 28, 2017, at Premier's Breakthroughs conference. The following Cares Award finalists were also recognized:

The goal of the Roseland Community Initiative is to ensure children with the chronic disease asthma have access to a specialist as often as needed. The Initiative is reducing ER visits, hospitalizations, and school absences for children with asthma. To accomplish this, Mobile Care Chicago stationed a Certified Asthma Educator in Roseland Community Hospital's ER to speak with families and move them to on-going, comprehensive specialty care aboard Mobile Care Chicago's Asthma Van, a specialty mobile clinic. Asthma Educators also worked directly with local schools to proactively reach families before asthma episodes translated into ER visits. The Initiative has reduced pediatric asthma ER admittances at Roseland Hospital by over 80 percent and school absenteeism by 37 percent.
