CAST Releases New Tools for Selecting Shared Care Planning and Coordination Technology

WASHINGTON, Oct. 20, 2016 /PRNewswire-USNewswire/ -- The LeadingAge Center for Aging Services Technologies (CAST) has released a new portfolio of tools designed to help long-term and post-acute care organizations select shared care planning and coordination technology.

The portfolio of tools includes:

  • A whitepaper that helps organizations understand the range of issues surrounding shared care planning and coordination, outlines technology solutions available in the marketplace, as well as their uses and benefits, and assists providers in planning for these technologies.

  • An interactive guide that is geared to busy executives who want to quickly navigate the comprehensive whitepaper and get a high-level overview of the issues, technologies, and planning process covered.

  • A Product Selection Matrix and Online Selection Tool. The matrix compares 18 different products from 17 vendors. The easy-to-use online selection tool helps providers identify the appropriate shared care planning and coordination tehnology products that fit their care settings and offer their "must-have" features.

  • 5 case studies that provide real-life examples of how long-term and post-acute care organizations successfully implemented shared care planning and coordination technology.

Shared care planning and coordination involves creating a care plan, a personalized written statement of a care recipient's comprehensive health and social support assessed needs. It sets out what support providers and caregivers should deliver, why and when, and who is meant to provide it.

The ability to communicate with, and educate, patients in their home is an important feature for care management. Technology that accomplishes and/or facilitate these goals can provide substantial benefits. Care planning and coordination technologies can educate patients and help them manage chronic diseases, support and stabilize post-acute patients, ease care transitions, and coordinate support services for older adults.

"Shared care planning and coordination technologies can help providers improve communication, quality of care, and patient engagement and satisfaction," said Majd Alwan, Ph.D., LeadingAge senior vice president for technology and executive director of CAST. "We're excited to provide these tools to make true person-centered care easier and more efficient."
