This year's list measures revenue growth from 2011 to 2014. To qualify, companies must have been founded and generating revenue by March 31, 2011. Additionally, they must be U.S.-based, privately held, for profit, and independent by December 31, 2014. The minimum required 2011 revenue is $100,000 and $2 million in 2014.
