CareCentrix Participating in Panel Discussion on Reducing Readmissions at Customer Analytics & Engagement in Health Insurance Conference

CareCentrix, the leader in managing care to the home, announced that Beverly Hansen, Vice President of Medicare and Risk Product Development, will serve as a panelist today at the Customer Analytics & Engagement in Health Insurance conference. The event is taking place in Chicago and is being hosted by FC Business Intelligence.

Hansen will discuss the ability of analytics, enabled by technology, to drive better patient care, lower costs and improve outcomes. She is joined on the panel by Dr. Robert Furno, Chief Medical Officer for CMS; Griselda Chapa, Adjunct Faculty for Health & Wellness at Tulane University; and Richard Proctor, General Manager for Healthcare at Hortonworks.

"We have a wealth of data and new technology available to us to help address readmissions in a meaningful way," said Hansen. "However, what's important for all of us to remember is to keep the patient at he core of the work we do."

CareCentrix uses a patient-focused approach in its HomeSTAR program, which has reduced hospital readmissions by nearly 20 percent. HomeSTAR, which stands for Successful Transition and Recovery at Home, is a proprietary transitional-care program designed to meet the unique needs of patients as they are discharged from a hospital or skilled nursing facility.

By assessing patients' current conditions, as well as their medical history, the program is able to evaluate patients' risk of readmission and take steps to prevent it. The program also has a high patient satisfaction rating with 95 percent of participants saying they would recommend HomeSTAR to a family member or friend.
