SCAN Health Plan Announces Medicare Advantage Benefits; Expands Into Two New California Counties for 2015


LONG BEACH, Calif. --(Business Wire)--

SCAN Health Plan announced today its 2015 benefit plans, designed to provide Medicare-eligible Californians easy access to providers and programs that support healthy and independent aging. SCAN is one of the nation's largest not-for-profit Medicare Advantage plans.

In 2015 SCAN's competitive benefit plans will continue to be available to Medicare-eligible beneficiaries in Los Angeles, Marin, Orange (News - Alert), Riverside, San Bernardino, San Diego, San Francisco, San Joaquin, Santa Clara, and Ventura counties. In addition, beginning January 1, 2015, SCAN will be available in Napa and Sonoma counties.

SCAN members receive all of the basic benefits traditionally covered under Medicare as well as a prescription drug benefit (Medicare Part D), while paying no deductibles and lowco-pays for most services. Other benefits may include vision, routine transportation and hearing aids.



SCAN's Medicare Advantage plan, SCAN Classic (HMO), is available in all counties SCAN serves. In addition, in ten California counties (Los Angeles, Marin, Napa, Orange, Riverside, San Bernardino, San Diego, San Francisco, Santa Clara and Sonoma) SCAN is offering Special Needs Plans that are available to eligible individuals with chronic conditions - SCAN Heart First (HMO SNP) is for people with cardiovascular disease or congestive heart failure, and SCAN Balance (HMO SNP) is for individuals with diabetes. Both offer the same benefits as the SCAN Classic plan as well as additional benefits specific to managing heart disease or diabetes. In four counties (Los Angeles, Riverside, San Bernardino and San Joaquin) SCAN Connections is available to individuals who are dually eligible for both Medicare and Medi-Cal.

The annual enrollment period for selecting a Medicare Advantage plan for 2015 runs from Oct. 15 to Dec. 7, 2014. During this time seniors and others on Medicare may choose to switch their health plan or move from traditional Medicare to a Medicare Advantage plan (or move back to Original Medicare) with their new elections taking effect Jan. 1, 2015.

SCAN Health Plan is an HMO plan with a Medicare contract offered in California by SCAN Health Plan and in Arizona by SCAN Health Plan Arizona. Enrollment in SCAN Health Plan depends on contract renewal. SCAN Balance (HMO SNP) is available to anyone with Medicare who has been diagnosed with diabetes. Heart First (HMO SNP) is available to anyone with Medicare who has been diagnosed with certain cardiovascular conditions.
