Rise Health Builds Competitive Advantage for Integrated Health Partners

"Through Stratus, we can drill down to the patient level to identify high risk patients and those who are more likely to have an ER visit or who may have a negative health outcome in order to create individual care plans for our patients." As IHP expands into new offerings, the physician hospital organization will require sophisticated reporting for administrators to help manage and validate the impact that programs are having on the target patient population.
