Qualidigm Announces New Board of Directors Members for 2014 to 2016 Term

He is also the medical director of Hartford HealthCare’s Medicare Shared Savings Program, which currently has more than 15,000 patients in a Medicare/CMS shared-risk pilot program that began one year ago. Dr. Carroll is board certified in Family Practice and in Adolescent Medicine. Columbia University recognized Dr. Carroll for his work in mental health screening for Navajo adolescents at the TeenScreen National Center and received the Area Directors’ Award for Outstanding Clinical Leadership for Navajo Area Indian Health Service Department Development of School Based Health Clinic Network Navajo Research.



Kate Emery is the founder and CEO of Walker Systems Support, a Farmington-based technology firm offering network management, IT project support, security services, IT staffing and Web/interactive services. Walker, one of the largest computer consulting firms in Connecticut, restructured as a Social Enterprise in 2007 with one third of distributed profits going to employees, one third to community, and one third to shareholders. Emery also founded the Social Enterprise Trust -- "reSet" -- which has created a social enterprise incubator at 99 Pratt Street in Hartford, and established a Social Enterprise Investment Fund to help start or expand social enterprise in Connecticut. Emery also serves on the board of the Boy's and Girls Club of Hartford and the Connecticut Technology Council.

Molly Rees Gavin is president of Connecticut Community Care, Inc. (CCCI), a statewide nonprofit care management organization serving more than 9,000 individuals and their families through the Department of Social Services Connecticut Home Care Programs, Money Follows the Person and CCCI's private division, Care Management Associates. She is certified by the National Academy of Social Workers. Rees Gavin serves as the president of the National Academy of Certified Care Managers, an Executive Committee member of the Board of Directors of the Alzheimer Association's CT Chapter and the Institute for Senior Living Education. Additionally, Rees Gave serves on the Connecticut Elder Action Network Executive Committee and co-chairs the Department of Social Work Community Advisory Board at the University of Saint Joseph in Hartford.


Frances G. Padilla is president of the Universal Health Care Foundation of Connecticut. As former executive vice president there, she spearheaded the Foundation’s research and policy initiatives, leading to the design of the landmark 2009 SustiNet Law and health care reform legislation passed in 2011. Under Padilla's leadership, the Foundation employs an activist philanthropy approach to achieving universal access to quality and affordable health care for Connecticut residents. She has ushered the creation of an innovative funding program to build a movement for universal health care through results-oriented partnerships in education, outreach and mobilization. Padilla serves on the Governor’s health reform board, co-chaired by State Comptroller Kevin Lembo and State Healthcare Advocate Victoria Veltri. Padilla is also on the board of the Center for Assessment and Policy Development, a national social policy think-tank. She is a former member of the National Partnership for Action Region 1 Health Equity Council, a U.S. Dept. of Health and Human Services initiative. Padilla co-founded the Community Foundation of Greater New Haven’s Progresso Latino Fund. She served two terms on the New Haven Board of Education, the boards of The Hospital of Saint Raphael and the Community Loan Fund of Greater New Haven.

Steven Wolfson, M.D. is associate clinical professor of Medicine at the Yale University School of Medicine. He is the founding partner of Cardiovascular Associates of New Haven, P.C. Dr. Wolfson is a Fellow of the American College of Cardiology, the American Heart Association Council on Clinical Cardiology and the American College of Physicians. He is New Haven County’s councilor to the Connecticut State Medical Society, and the former medical director of the New Haven Community Medical Group. As a founding member of Project Access-New Haven, a nonprofit organization established to provide free care to New Haven’s uninsured, Business New Haven named Dr. Wolfson a 2014 Healthcare Hero. He is a frequent contributor to the Op-Ed pages of Connecticut’s two largest daily newspapers.
