TriZetto Corporation today announced that a highly specialized Medicaid care coordination plan in Washington, D.C., has selected integrated technology and services solutions from TriZetto (News - Alert) to enhance the delivery of benefits and care to its members and support the plan's intended expansion into new markets.

Health Services for Children With Special Needs, Inc. (HSCSN), serves 5,400 children and youth in the District of Columbia. Its members have a range of life challenges including high-need social determinants of health, mental health disorders, developmental disabilities such as autism, and severe medical conditions such as cerebral palsy and cystic fibrosis.

Chapman said that HSCSN's holistic approach to member benefits and care relies on more than just clinical information. A "home environmental scan" is conducted to check for issues such as lead in the home and to gauge family support and resources.

"TriZetto's integrated enterprise administration and care management platform will allow our care managers to track these highly important social determinants, accurately stratify our population and provide tailored interventions and member communications that can improve clinical outcomes and quality," said Chapman.

TriZetto's enterprise administration and care management systems scale to growth, and this figured in HSCSN's selection of the solutions over competitor products, according to Rob Scavo, president of the regional accounts market for TriZetto. "HSCSN intends to leverage its specialized, niche expertise to expand into other metropolitan areas and partner wit healthcare payers that serve young adults with disabilities and chronic illness. This health plan is poised for growth, and TriZetto's technology platform will scale to it."



With its plans for growth, and with more than 60 percent of its employees performing care management functions, HSCSN opted for application management services from TriZetto.
