Magellan Behavioral Health of Pennsylvania Releases Report to the Community
AVON, Conn. --(Business Wire)--
Magellan (News - Alert) Behavioral Health of Pennsylvania, Inc., part of the Magellan
Healthcare division of Magellan Health, Inc. (NASDAQ: MGLN) today
released its second Report to the Community. The report highlights the
work Magellan, working closely with Bucks, Delaware, Lehigh, Montgomery
and Northampton counties, has done on behalf of the 270,000
Pennsylvanians the counties and Magellan serve annually. Magellan has
managed behavioral health services for HealthChoices members for the
past 17 years.

customers and providers serve as the foundation for the impactful
programs we are able to provide to HealthChoices members," said Diane
Marciano, general manager of Magellan Behavioral Health of
Pennsylvania's Lehigh Valley Care Management Center. "This report offers
details about the work we've been able to do together and highlights
several particularly innovative and successful programs across our
service area as we seek to facilitate and accelerate the transformation
of the behavioral health system, and support individuals and families on
their journey toward wellness."

have benefited from Magellan's services. Included among the highlighted
programs are:
Neighborhood
Health Centers of the Lehigh Valley/Haven House Integrated Care

Magellan, Neighborhood Health Centers of the Lehigh Valley (NHCLV) and
Haven House together are delivering an enhanced model of integrated
primary care and behavioral health, the first co-location model in Lehigh
County. This coalition provides comprehensive and coordinated care
for individuals with mental health and physical health challenges by
offering a community-based, personalized approach with fully
integrated behavioral health, case management and primary care.
Transition
to Independence Process (TIP)

In Bucks County, transition-aged youth are benefiting from the
Transition to Independence Process (TIP) model, the only
evidence-supported best-practice model in the United States for
individuals in this age group with emotional/behavioral difficulties.
The program is designed for youth ages 18 to 26 who are living with
mental health challenges and/or intellectual disabilities. The program
aims to engage youth in their own future planning process; provide
youth with non-stigmatizing, culturally competent and appealing
services and supports; and help youth and their families move toward
self-sufficiency and achievement of goals in employment/career,
educational, living situation, well-being and community-life domains.
Consumer
Recovery Investment Fund/Self-directed Care

A partnership between Delaware County, the Mental Health
Association of Southeastern Pennsylvania and Temple University, the
Consumer Recovery Investment Fund (CRIF), a self-directed care
project, is helping adults with serious mental illness and a history
of steady engagement in behavioral health services find the road to
recovery. With the help of a recovery coach, who is a peer specialist,
individuals develop a person-centered, recovery-oriented care plan
that includes both traditional, in-plan services and non-traditional,
out-of-plan goods and services, the latter of which are paid for by
"Freedom Funds." One-hundred percent of participants would recommend
the program to a friend, and 98 percent said they felt that being
involved with this program facilitated their recovery.
Adult
Mobile Crisis Program

In 2012, Montgomery County and Magellan identified the need for
a locally focused, enhanced adult mobile crisis program to help
individuals in crisis avoid hospitalization where possible.
Individuals who are in crisis and at risk of hospitalization are
identified and screened for levels of risk by provider Access
Services. If risk is not imminent, the individual is diverted to the
appropriate community supports and providers. If hospitalized, the
person receives a discharge plan that includes engaging with the
mobile crisis service in order to stay safe. By the end of 2013,
outcomes indicated a 97 percent diversion rate for individuals having
had a telephone and/or mobile crisis intervention.
Extended
Acute Care Services

When Allentown State Hospital closed in December 2010, Magellan
collaborated with the state, Lehigh and Northampton counties,
local providers and community members to plan alternative services and
implement successful extended acute care programs (EACs) in both
counties for affected residents with serious and persistent mental
illness. Focusing on recovery skills for successful transition to the
community, most individuals are discharged to community-based services.
