Health Care Advantage Teams up With Partners in Primary Care


Health Care Advantage (HCA), a company dedicated to improving the health and wellbeing of seniors with Medicare, announced it has recently added the physician group, Partners in Primary Care (Partners), to its partner network. Together, the two companies are working to spread awareness about the benefits of senior-focused care.

Staffed with board-certified physicians, along with care teams specially trained to treat seniors, each location offers a range of convenient services including dedicated care teams, physician services, treatment, laboratory services, immunizations and chec-ups in one location.



Partners incorporates a proactive care model and works with patients and their families to provide support, including reminding patients of screenings and exams, thoroughly explaining medical information and advocating for patients when communicating with specialists, hospitals and Medicare plans.

Partners also helps their patients coordinate care with hospitals and specialists, making the healthcare system easier to navigate. Additionally, Care Coaches are available at each center, providing extra support to patients after hospitalizations or for those who need support with managing certain chronic conditions. The centers also include an in-house pharmacy that offers patients medication coordination and prescription cost management; and an activity center that offers health education classes, fitness activities and classes from community partners focused on the senior population.
