Presented are examples of successful patient engagement results including lowering the percentage of diabetes patients with hemoglobin A1c greater than nine. By analyzing data in the patient registry, Borgess Health discovered many patients hadn't had their hemoglobin A1c checked in over 12 months. These patients were quickly enrolled in care management programs, and within three months, their hemoglobin A1C greater than nine dropped from 30 to 19.6 percent.
