Israel's drug market expenditure was estimated at ISL6.2bn (US$1.5bn) in 2007. Over the forecast period (2008-2012), BMI expects the market to grow at an average of 3.3% year-on-year to reach ISL7.2bn (US$2.4bn) in 2012. Growth measured in US dollars should be stronger due to the likelihood of further strengthening of the Israeli shekel against the US dollar. In BMI's updated Business Environment Ratings for Q408, Israel is ranked third out of 15 Middle East and Africa markets surveyed with a score of 54. The country receives the highest score in the region for potential returns, due to the large market size by regional standards and high per capita drug expenditure. Israel's risks to realisation of potential returns are relatively severe. The country's regional isolation creates a moderate degree of political risk. However, the most serious risks are pharmaceutical industryspecific due to a slow approvals process, intellectual property environment that continues to draw criticism from abroad and unclear reimbursement regime. The lack of clear methodology underpinning Israel's reimbursement regime was further highlighted in September 2008 after the Ministry of Health seemingly bowed to patient pressure by adding a screening test performed on couples to detect whether they carry a genetic mutation that would lead to a risk of having children with cystic fibrosis in the basket of health services. The mutation associated with cystic fibrosis is common among Ashkenazi Jews. However, the Ministry of Health had previously resisted its inclusion. In August 2008, Israel was reportedly close to joining the Pharmaceutical Inspection Convention and Pharmaceutical Inspection Cooperation Scheme (PIC/S), following a successful check of pharmaceutical manufacturing standards in a joint inspection by the Ministry of Health's Institute for Monitoring and Standards and representatives from PIC/S. In the near term, an agreement of mutual recognition will be signed between Israel and the EU, recognising the respective drug manufacturing oversight by regulators. PIC/S membership should further boost Israel's pharmaceutical exports, which continue to underpin the strength of the domestic pharmaceutical industry. According to the Manufacturers' Association of Israel (MAI) pharmaceutical exports reached US$2.22bn in H108, a 43% increase on H107. Such figures suggest exports alone are more than twice the value of domestic drug consumption.
