Netsmart Reveals Innovative Digital Technology to Advance Value-Based Care Delivery for Home Care and Hospice Agencies at NAHC 2022

Netsmart will reveal the latest solution and software innovations of the CareFabric® platform that are transforming care delivery for home care and hospice agencies at the National Association for Home Care and Hospice (NAHC) Conference and Expo on Oct. 23-25 in St. Louis.

NAHC attendees can learn about new technology capabilities to support value-based care programs by leveraging the CareFabric platform to support all post-acute service lines on a single platform, including palliative care. Netsmart has been at the forefront of advancing the next generation of palliative care capabilities by achieving the industry's first 100% certification of base criteria, earning the myUnity® electronic health record (EHR) certification from the Office of the National Coordinator for Health Information Technology. By earning ONC certification, Netsmart has the ability to equip post-acute care providers with a platform to operationalize and manage large-scale palliative care programs, while qualifying for reimbursement to participate in CMS MIPS/MACRA programs.

"Home care and hospice providers are facing obstacles unlike ever before," said Netsmart SVP & Managing Director of Post-Acute Dawn Iddings. "Having a committed technology partner to help you deliver high-quality care is the key to future success. As providers navigate the growing demands of the market, attract and retain qualified staff and keep up with the ever-changing regulatory requirements, digital transformation is needed now more than ever. Our teams have worked alongside our client partners to develop new functionalities and we look forward to demonstrating ways our cutting-edge solutions are helping post-acute care clients digitize and integrate their operations across a variety of functions."

By visiting Netsmart at booth 915, attendees can discover how organizations are leveraging the population health management solution, CareManager™, to improve care coordination between providers across healthcare and CareRouter mobile dispatch technology to optimize visit assignments, enable in most instances real-time visibility to staff and enable caregivers in the field to respond quickly to crisis calls. The easy-to-use CareRouter solution was developed in collaboration with long-standing client partner, Ohio's Hospice, who has increased speed to care by 90% since implementation and achieved their goal of dispatching care within 15 minutes of receiving a request.

The CareManager technology is advancing post-acute care organizations' population health management initiatives, enabling providers to reduce gaps in care, avoid risks during care transitions, lower readmissions and better manage value-based contracts. Clients utilizing CareManager have experienced up to a 10% reduction in annual costs for value-based contracts, a 40% reduction in time spent coordinating treatment across partner providers, 50% less time spent on caseload management, and up to 80% less time spent on double entry of clinical data.

Netsmart will showcase one of the latest additions to the CareFabric integrated platform, a cloud-based revenue cycle management solution that helps healthcare providers reduce account receivable collection hours by as much as 20% while accelrating cash flow, improving their productivity and increasing their profitability. As organizations face staffing challenges, the collections management automation solution increases staff efficiency, productivity and drives end-user satisfaction.

NAHC attendees can view how automated referral technology is leveraged to position your agency to gain more referrals and boost revenue. Netsmart Referral Manager® is a web-based, EHR-agnostic referral solution that simplifies the acceptance and rejection process, enabling providers to handle referrals efficiently and effectively. Referral Manager helps address staffing shortages by reducing the need for emails, phone calls, repetitive tasks and manual data entry reducing time spent by 50% on the referral process. The benefits experienced by clients include achieving up to a 40% reduction in time spent on referral admission processes, a 70% reduction in time spent processing referrals into the EHR, and a 10% reduction in annual costs of care for shared risk programs.

Providers can learn about the upcoming launch of the Netsmart Simple™ home health offering for scrubbing and transmitting OASIS data and request a demonstration of a variety of CareFabric solutions that drive efficiencies, reduce administrative burden and improve the delivery of care.
