The program will share lessons learned and proven solutions both nationally and globally. As part of its Springboard Health Initiative, Geisinger also launched the Fresh Food Farmacy, which addresses health challenges faced by patients with diabetes and food insecurity. The goal is to improve access to both healthy food and nutritional knowledge. The target population has high rates of obesity, pre-diabetes, and diabetes, and early results show significant improvements in blood sugar control, weight, and overall health management.
