Allscripts Care Management Joins With CarePort Health to Build Next Generation Care Coordination Solutions


CarePort Health, an Allscripts company and pioneer in post-acute outcomes management, today announced it is now managing the Allscripts (NASDAQ: MDRX) Care Management solution and services portfolio. This transition is part of a strategy to accelerate innovation of the Care Management platform. Together, the Care Management and CarePort platforms offer providers, payers and ACOs visibility across all levels of care and transitions across the continuum. The newly expanded CarePort organization will debut at the Annual Case Management and Transitions of Care Conference (ACMA) 2018 in Houston, TX on April 24-27.

Founded as ECIN in the mid-1990's, the Care Management solution is now a leader in acute case management and post-acute referral management. Its EHR-agnostic platform is used by nearly 1,000 hospitals across the country:

  • Care Management - A complete Acute Case Management, Discharge Planning and Utilization Review workflow tool with a built-in network of more than 20,000 providers.
  • Referral Management - An intake, referral management and discharge planning tool for post-acute care providers.

Moving forward, the CarePort suite of products include CarePort Care Management, CarePort Referrl Management, CarePort Guide, CarePort Connect and CarePort Insight.



The coming together of Care Management and CarePort represents a natural progression given the current industry trends. "The shift to outcomes-based reimbursement models created a heightened need for providers and health organizations to transition patients out of the hospital efficiently with established solutions, such as Care Management, and to track patients' health as they transition across multiple care settings, using technologies such as CarePort," said Dr. Lissy Hu, chief executive officer and founder of CarePort Health. Multiple senior executives with a deep knowledge of Care Management have been recruited to join CarePort's executive team. Scott Kerber, co-founder and CTO of ECIN, will lead development and solutions. Mark Heron, a sales leader with a 18-year history with Care Management, is leading the sales organization. Dave Borowski, who has a 14-year history with Allscripts and ECIN, including project management and services across the population health suite, is leading Services and Support.

Today's announcement further exemplifies CarePort Health's accelerated growth since its inception in 2012. After its acquisition by Allscripts, CarePort Health has organically grown its customer base of leading medical centers, accountable care organizations (ACOs), and hospitals across the United States. Inside Allscripts, CarePort, including Care Management, is growing to 200 full-time employees by the end of 2018. The Boston-based team recently moved into a new, larger office space to accommodate its expanding team.
