Health Dialog Achieves 15 Consecutive Years of NCQA Accreditation for Disease Management Programs

Health Dialog, a leading provider of integrated population health management solutions for health plans, risk-bearing providers and self-insured employers, announced today that it has received Patient Oriented Accreditation from the National Committee for Quality Assurance (NCQA) for its Disease Management programs for asthma, congestive heart failure, chronic obstructive pulmonary disease (COPD), coronary heart disease and diabetes. NCQA's most recent Accreditation represents 15 consecutive years of Accreditation for Health Dialog's Disease Management programs, securing its place as an industry leader in helping organizations manage care for people with chronic diseases. NCQA's most recent award extends the Accreditation period until October 27, 2020.

"NCQA's Disease Management Accreditation program is thorough and rigorous," said NCQA President Margaret E. O'Kane. "It's designed to highlight only those programs that truly improve chronic care."

NCQA's Disease Management Accreditation demonstrates that a disease management program is dedicated to giving patients the support, education and other help necessary to facilitate good patient outcomes and care. NCQA awards Accreditation to organizations that meet or exceed its rigorous standards and guidelines. The standards are set intentionally high to help organizations achieve the highest level of performance possible, increase adherence to care guidelines and create an environment of continuous quality improvement.

"Fifteen consecutive years of NCQA Accreditation is an incredibly powerful validation of the quality of our Disease Management programs for asthma, congestive heart failure, COPD, coronary heart disease and diabetes," said Danielle Barrera, Chief Operating Officer for Health Dalog. "We're continually improving our integrated suite of population health services, including our Disease Management programs, so that organizations can better engage members in their health journey, which can help them support better health outcomes for members and achieve the significant cost reductions our clients have enjoyed for more than two decades."

Health Dialog's disease management solutions provide personalized interventions that help individuals better manage chronic diseases and reduce healthcare costs. Beyond achieving NCQA Accreditation for five major chronic diseases, Health Dialog's solutions address more than 65 comorbid and preference-sensitive conditions. Programs range from intensive chronic disease management to wellness improvement and shared decision making -delivered through telephonic health coaching, digital engagement, and health education tools and resources. Our team of nurses, respiratory therapists, dieticians and health and wellness coaches use Health Dialog's proprietary HDC3™ platform, a sophisticated coaching workflow tool to enable seamless integration with the health plan or provider. Health Dialog also offers a 24/7/365 telephone nurse line that, unlike callback services, provides immediate, live access to a registered nurse. All of Health Dialog's Chronic Care Management solutions are backed by the company's powerful analytics that stratify a population based on key risk indicators to enable highly personalized, effective interventions.
