Turn-Key Health Presents Role of Specialty Palliative Care to Empower Patients and Families at AHIP 2017 National Conference on Medicare

She points out the value of a more personalized, high-touch approach to care that is provided by community-based palliative care teams.

"These nurses and clinical social workers are experts at conducting in-home and telephonic assessments, providing pain and symptom management, medication reconciliation, care coordination, goals of care discussions and advance care planning," she continues. "These specially trained professionals are available in virtually every community, allowing payers to increase the scope and frequency of touching individual patient lives, match appropriate resources to patient needs, and scale their programs for larger populations. It's a model that delivers high quality care and improved health at lower total cost."
