Therap Introduces Enhanced Incident Reporting Tool


WATERBURY, Conn., Sept. 12, 2017 /PRNewswire/ -- Therap announces an improved and user-friendly interface for its incident reporting tool or General Event Reports (GER) module. User experience is enhanced through a simple and intuitive design. The new design maintains the original functionality of the module while streamlining the number of steps needed to complete the task.

The Incident Reporting tool on Therap's disability software is used by agencies nationally to store and track incident reports on individuals they are providing support and care to. The incident reporting tool is available for documenting reportable events including serious injuries, medication errors, allegations of abuse, neglect or exploitation, missing person, death, behavior incidents, and other critical events. This tool is customized to support different state requirements and allows multiple related incidents to be recorded on one form. Medication errors can be logged along with notes that serve to convey review comments to the staff who filed the original report. The incident reporting eatures maintain multi level access control so that only supervisors and QA directors are able to read reports containing allegations of abuse, neglect or exploitation and names of staff responsible for medication errors.



Therap's state specific incident reporting feature is using across multiple states to comply with state regulations. Therap's state forms are designed to conform with state policies and are accessed only by users of that particular state. Users automatically generate the state's incident report which can then be printed for submission to the designated state department. Therap's state specific incident reporting forms including New York OPWDD Incident Reporting Forms (OPWDD 147, 148, 150, Jonathan Law Notification and Follow-up Information), Connecticut DDS-Incident Report (255, 255 OH/Fam, Medication Error Report 255m), Florida Agency For Persons with Disabilities (APD) (Initial, Follow-up, Medication Error) and more.
