Humana uses the CDC's population health management tool known as Healthy Days to benchmark community health and measure progress. Lessons Learned Through its Bold Goal program, Humana has found that making communities healthier demands an integrated approach. Creating lasting behavior change in the communities Humana serves also requires highly local and holistic care solutions that focus not just on clinical measures but social determinants of health. This demands a deep understanding of the fabric of local communities. This approach, which Humana has emphasized in it Bold Goal communities, specifically addresses how social determinants such as food insecurity, health literacy and transportation can significantly impact an individual's health. For example, 5.4 million seniors in the U.S. are food insecure, and 43 percent feel lonely on a regular basis. Key Regional Findings The 2017 Bold Goal Progress Report details Humana's collaborative efforts with physicians and community partners in places such as San Antonio, Louisville, Tampa Bay, Broward County (Florida), New Orleans, Baton Rouge and Knoxville. Roy A. Beveridge, M.D., Humana's Chief Medical Officer, whose organization is responsible for leading the implementation of the Bold Goal, noted the significant impact that Humana and its physician and clinician partners have played in these local communities. Moving Forward Thanks to the support of physician leaders, businesses, nonprofits, and government and public health organizations, Humana continues to make progress toward its 2020 Bold Goal. More Healthy Days are being experienced nationally, and new programs addressing both local health issues and social determinants of health have been launched. Moving forward, 2017 priority issues for Bold Goal communities include diabetes, behavioral health, food insecurity, loneliness and social isolation. Key learnings from local programs enacted to address these challenges will be expanded nationally. Humana is also evaluating new communities to expand its Bold Goal program.
