Medicaid Providers Achieve Better Outcomes Using A Common Care Plan


BEAVERTON, Ore., July 27, 2016 /PRNewswire/ -- Medicaid providers pursuing healthcare quality improvement initiatives face complex challenges when addressing the needs of high-risk patient populations.   • Central City Concern, an FQHC in Portland, Oregon, utilizes Enli CareManager across its clinical network to help providers identify and close gapsat the point of care, including those of its diabetic patient population.   • Contra Costa Health Services serves the health of all people in Contra Costa county, with special attention to those most vulnerable. Their facilities support over 640,000 visits annually for medical care, behavioral health, and other concerns. Enli CareManager is a central pillar in Contra Costa's primary care redesign strategy.
  • Fenway Health is a FQHC in Boston, Massachusetts that manages a patient population of more than 24,000 individuals with a staff of 500. Among other priorities, Fenway is using Enli software to help address the healthcare needs of the LGBT community.



Established as a benefit under Medicare in October of 1991, FQHCs are safety net providers that primarily provide primary care services in medically underserved urban and rural communities.
