Cordata and PatientBond Partner to Improve Specialty Care Coordination with Personalized Communications

Cordata Healthcare Innovations today announced a partnership with PatientBond, LLC. PatientBond's targeted, personalized communications toolset will be embedded into Cordata's specialty care coordination platform and available for Cordata users.

"This partnership benefits our hospital and health system clients in multiple ways, from focusing care coordination resources on the interactions that matter most, to helping patients overcome barriers to care and stick to their treatment plans," said Gary Winzenread, CEO of Cordata. "This capability is especially useful for care coordination programs that feel understaffed and overwhelmed. Beyond making coordinators more productive, it also helps hospitals develop and sustain strong and personalized relationships with people needing care for complex or chronic conditions."

"Patient engagement is a worthy goal, but meaningful behavior change must be the target to improve patient outcomes and reduce outmigration," said Casey Albertson, President and Chief Operating Officer, PatientBond. "Partnering with Cordata will allow us to change the behavior of many more eople. Thanks to Cordata's focus on critical specialties, the partnership will also have a significant impact on hospital profitability, both today and in future risk-based payment models."

Cordata's specialty care coordination platform helps healthcare organizations improve care for people with chronic specialty diseases through effective patient management and retention along complex care paths. The enhanced communications capabilities will support critical features within the current Cordata platform, including risk scoring, medication and appointment adherence for oncology, spine and behavioral health specialties.

Automated tracking and validation of patient behavior free coordinators to focus on exceptions. For instance, because coordinators receive notifications if patients skip an appointment, coordinators no longer need to manually validate the compliance of all patients, a time-consuming and relatively low-value task.

Winzenread added, "PatientBond and Cordata share a passion for open collaboration and addressing the 'human factor' to improve healthcare. Automating communications makes care coordinators more productive and personalizing communications for patients makes them more effective. This is the way to drive both better clinical and business outcomes in healthcare."
