LUMEDX Corporation, a leading provider of cloud-based cardiovascular information systems (CVIS), and Continuity Health Solutions, maker of Ally® Connected Care and other outpatient disease management software, announced today that they have partnered to offer healthcare providers an integrated, longitudinal record for heart failure populations across the inpatient and post-acute care environments. Leveraging Ally® Connected Care platform embedded into LUMEDX's HealthView CVIS, patients can perform daily, personalized self-assessments consisting of biometrics, medication compliance, education and questions that are automatically transmitted to their electronic health record.

The new solution will be unveiled at the American Association of Heart Failure Nurses Annual Conference, LUMEDX Booth 106, in Los Angeles, June 26-28, 2014.
