Growing Demands to Coordinate Population Health Management Increases PerfectServe's Users by 50 Percent


Adoption of the population health model of care is increasing, and with that means larger care teams spanning across more care settings that may not be equipped with the technology or processes to ensure providers remain connected outside the four walls of a hospital.
