Post-acute facilities care for patients at a critical point in the care continuum. According to research conducted by the University of Colorado School of Medicine1, approximately 23 percent of patients discharged to post-acute care facilities following acute hospitalization were readmitted to the hospital at least once within 30 days. Nearly half of these readmissions occurred within two weeks of discharge.
